Hundreds of children and young people are in care across the Tees Valley area right now. By volunteering as a mentor you could be that crucial role model who helps a young person build confidence, life skills and self-esteem.

Thank you for taking this important step in helping to make a difference to the lives of children in care. This form should take no longer than 5 minutes to complete. This is the first step to becoming a volunteer.  Following this we will contact you to arrange an interview.

DETAILS ABOUT YOU

It’s helpful if you can provide us with at least 5 years’ employment, volunteering or educational history now. However we can collect this from you over the phone when we arrange an interview with you if easier.

References
Due to the sensitive nature of our volunteer roles and organisations, we have safer recruitment procedures in place. Our volunteering policy means we will be unable to accept volunteers without doing reference checks.

If you don’t have all of the referees’ full contact details to hand you can give us as much information as you can now. We will need the rest before we interview you. Referees can include current or past employers, teachers/lectures/tutors, supervisors from previous volunteering, extra-curricular leaders (e.g. scouts/church/sport clubs). We’re sorry but we can’t accept friends or family as references. You can leave filling in your referees until a later point but it would be helpful if you could fill this in now as we are unable to progress to interview without them.

Protecting Children, Young People and Adults
We need and want to keep vulnerable people and children safe. Because of this, we have to ask some personal questions about criminal convictions (spent and unspent), cautions or pending/ongoing court cases. All paid and unpaid roles are exempt from the Rehabilitation of Offenders Act 1974. The amendments of the Exceptions Order 1975 (2013) provide that certain spent convictions and cautions are 'protected' and are not subject to disclosure to employers and cannot be taken into account. Having a criminal record does not necessarily stop people helping out with Changing Futures NE. We will look into each case individually.

If you have any offences against children or vulnerable adults, or aren't honest with us on the form, you won't be able to volunteer with us. If you are barred from working with children or vulnerable adults and make an application to us, we will report this to the police.

If you have ever been convicted of a criminal offence, cautioned, reprimanded or given a final warning by the police or have any court cases pending, please give details below (including dates of offences, sentences, cautions, reprimands, final warnings and details of court cases pending unless these are "protected" as defined by the Rehabilitation of Offenders Act 1974 (Exceptions) order 1975 (as amended in 2013)).
